Welcome Video:
This is a brief 2 minute video introduction to our office and location. (Watch more videos.)
Philosophy of Practice:
The philosophy of our practice is that of a conservative approach to your dental needs. It is our goal to preserve one’s own tooth structure as much as possible through tooth-conserving and minimally invasive principles and techniques.
It is also our goal to give our patients dentistry of the highest quality yet keeping in mind which treatment approach gives the best prognosis for long term success and the best value for your investments. The final treatment will ultimately be decided by you, our patients, but we will share with you the knowledge and the risks of alternate treatment options in order for you to make an informed decision.
We strive to deliver dentistry of the highest quality and at times we will require the assistance of specialty practices to insure you receive the best available treatment in all phases of your care.
As has been the case with so many of our patients, our intent is to establish long term relationships in an environment of mutual trust, respect, and satisfaction.
Get Acquainted Information
On your first visit we will require personal data and health background. This helps us complete insurance forms and other correspondence. The health information provides a baseline history to aid us in providing better dental care for you. These forms can be accessed with the patient forms link or you may click here.
Please inform us of changes in your name, address, phone numbers, or any changes in your health. Also, please inform us of any changes in your insurance coverage.
Appointments
It is necessary that we work by appointment. Our standard office hours are 8:00am to 5:00pm Monday and Wednesday, 7:00am to 4:00pm Tuesday and Thursday, and by appointment. If you are unable to keep a scheduled appointment please give us at least 24-hours notice so someone else may use your appointment time. Appointments rescheduled with less than 24-hours notice are subject to our standard appointment cancellation fee if we are unable to fill your time slot.
Conservation of time
We respect how valuable time is for all. In the interest of conserving our patients’ time, we complete treatment in as few visits as possible. We strive to have our appointments run on time and request cooperation in this respect. On the rare occasion we are late for an appointment it is often as a result of emergencies. In these situations we trust you understand the situation and know that in the event you experience an emergency, you too would receive the same priority treatment.
Telephone calls
Telephone calls received during regular office hours are handled by our staff. In most cases they will be able to answer your questions. For assistance after hours, please call our office and an emergency number will be provided. You may also leave a message and we can return your call the following day.
Use of email
For your convenience you may use email or our contact form to contact us regarding non-emergency matters. We monitor this account Monday through Thursday and will respond on these days within 24-hours of receipt of your email. Please keep in mind that messages sent via the contact form or via email will be read by our office staff, in addition to Dr. Werts and/or Dr. Yang. Please review our Appointment policy regarding rescheduling and we ask that you do not use email for emergency or urgent matters, please call (818) 241-1824.
Diagnosis
Using our examination findings, we will develop a treatment plan and recommend a program that will restore your mouth to optimal conditions. Recognizing that treatment plans and associated costs can vary widely, we present as many options as feasible and the advantages or limitations they would present. Above all, our treatment plans are conservative and both Dr. Werts and Dr. Yang take great pride that our office does not over-diagnose.
Professional fees
There can be different treatment plans for different circumstances. We discuss these options with you along with the tradeoffs, expected outcomes and required investments so you can make an informed decision. We ask for payment at the time services are rendered. We accept cash, check or credit cards (Visa, Mastercard, American Express, Discover).
We also realize that individual circumstances may require special payment arrangements. For your convenience we offer financing through CareCredit and we can assist you in the application process.
Insurance
Dr. Werts does not accept dental insurance or adhere to insurance fee schedules, but Dr. Yang does accept some PPO plans. We can bill your insurance carrier as a courtesy but patients are responsible for the portion that their insurance carrier does not pay. Payment is expected at the time of service unless arrangements are made in advance.
A Joint Responsibility
Total dental health is dependant on sound healthy tissue and bone. With the recent advances made in dentistry, we are able to give better care than ever before. When patients do their part in maintaining their oral health it is possible for the majority of patients to keep their natural teeth for a lifetime.
Surveillance
It is a joint responsibility to see dentistry is properly cared for after completion. Therefore, we maintain a recall system to notify you when to return for a prophylaxis and examination at regular intervals. If you have preference for specific times for these appointments it is recommended to make your next appointment at each visit.
Our goal is for our patients to maintain their oral health with an attractive smile in complete comfort.
